Is it possible for setup to copy a specified access database file only if it
does not already exist?

Yes.
See the Condition property on the file object. Read "Conditional
Statement Syntax" and you'll find the answer in the examples, I think.
I must admit, I'm not overly familiar with the Syntax... but I hope
that helps.
