Setting default form

I designed a new contacts form that is different from the
default. I want to import my address database from Excel
into this new format. However, when I try to import it, I
cannot select my new form as the destination, and it maps
the fields to the fields and the form of the original
default contacts setup. How can I make my newly form the
default that will accept imports?

(Note: I was able to set it up so that any new contacts I
establish use my newly designed form, by changing it in
the contacts property menu for "when posting to this
folder use..."

Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.slipstick.com/dev/customimport.htm.
