Set up Categories for a Contact, the Categories disappear. Help!

We assign different Categories for each Contact. The Categories literally
disappear. If we have 4 Categories assigned to a particular Contact, the
next day 3 of them will be gone. This is happening randomly, and not to all
the Contacts. Any idea why? Thank you.
