Can IIS be configured to use either
default.htm or index.htm or does it always have to be
index.htm
 
IIS can be configured to using any default filename, the normal is
default.htm or default.asp, it is just a matter of which is first in the
list.

To view/change, open Control Panel, Admin Tools, IIS, select web site, then
Properties, Documents

IIS can be configured to use an entire range through the default documents
listing in an IIS properties dialog's Documents tab. This can be set for an
entire virtual server, web, or even directory. Whatever is at the top of the
list gets brought up first though in case there is a conflict with several
available default files in the directory (like default.asp,
default.htm,etc.) FrontPage usually picks the very first one to use as the
name of a default file it creates.
