In the WinFax Send Fax Wizard, the third page is "Preparing the Cover
Page". Uncheck the box that says "Select a cover page..."

How can I edit/delete/create fax cover pages?

Using
Start > All programs > Windows Fax
there's a menu command
Tools > Personal Cover Pages
Clicking "New" opens a Fax Cover Page Editor

If Windows Fax isn't in your start menu, maybe you can find it in the
Control Panel under "Printers and Faxes."

Thanks, but as usual, I found this within 20 minutes of sending the post.

Funny how things so often turn up soon after you declare them lost.
