Search Form

I need to create a form where users can search for multiple data, and give an
end result. For example, a user needs to find a client by the last name SMITH
and lives in the city of CHICAGO and zip is 98765.

I've tried the Allen Browne site, but when I try to 'replace' some of the
criteria with mine, I get errors.
