Scans are just images, so you need software that extracts the text and other
images to produce an editable document. The precise formatting may be lost
in the conversion process but the software gets better every year.
Office XP and 2003 include the Microsoft Office Document Scanning/Imaging
tool which does that. although most scanners come with OCR (optical
character recognition) software. See, for example,
