same ol'.....form not sending email

I'm sorry if this is old hat to most of you. I have searched
everywhere but cannot find anything definitive.

I am working with four sites, all with the same ISP.

http://www.amdalinhome.com/jefftest.htm
http://www.avccc.org/jefftest.htm
http://www.heritagecare.org/jefftest.htm
http://www.nsmanage.com/jefftest.htm

I put the same page containing the form (jefftest.htm) on all four
sites and set the form properties on each site itself, rather than
publish it from the local sites.

The form only works on heritagecare.org.

I have checked myself 25 times at least (!) to make sure I set the form
properties the same. I have worked with the ISP admin and he insists
that everything is the same for all four sites.

Is there some Frontpage setting that I should be changing? Is there
something I can ask the administrator to check?

I'm at a total loss.
 
em addr it's sending to usually (mostly) has to be the same domain as the
website.
and the host has to allow use of the form handler as well.
