In Tools | Envelopes and Labels, click Options... on the Labels tab and
select Other/Custom. See if one of the Rotary Card definitions meets your
specs.

Couldn't find one in my Word 2003's
template list. Nor at MS web site.

Found literally dozens of Rolodex templates
by searching Google (www.google.com) for
"rolodex templates"

Suppose the dearth of Rolodex templates
is due to most people switching to
electronic contacts lists.

the Rolodex labels came with template instructions, so I can play with that.
 
Doesn't anyone want hard copy of their contacts anymore?
I like to have a rolodex nearby when the computer is not on. It does seem
like an antiquity. Amazing to me.

I didn't study them in detail, but Google displays dozens
of references to rolodex templates.

To answer your question, most people maintain their
contacts on PCs/Macs. Contacts are easily maintained.
provide quick access, and can be integrated with
other applications. Don't have a laptop but carry
portable contact list on Palm Titanium handheld.

As they say, to each his/her own.

I print an Outlook address book when I need a hard copy.
If you have another contact management program there is
probably a way of doing something similar. If I needed
Rolodex cards I would probably print labels and stick them
onto the cards. If your contact information is not set up
for mail merge or for printing labels you can type the
information into a table with cells the same size as the
labels.

The Rolodex cards came with a listing of default setting for a Word
Template, which I set; now I have to figure out how to export my address
books efficiently, which is a whole 'nother kettle o'fish.
