Right Click Menu on Desktop

While on the desktop if I right click to create an "New"
txt document the Text creation menu is gone. I can import
the settings into the registry and it appears but only
for one use. If I use the right click function, it only
lets me use it once then it disappears from the applet. I
can go back to the registry and recreate it time but this
is to time consuming. How could I fix this so it's back
to normal and will not disappear?

Kelly's site, below. Click on "N" and scroll down to...
"New Command - Add "NEW" Text file to the Context Menu"
 
well I tried kelly's suggestion but it did not work do you have any other suggestion or how could I write to
kelly and ask for help on this issue
