Report that shows record twice

I have a report that shows the questions an employee answered. However, it
shows the same response twice for each record. This happened when I tried do
use one query to do the work.
 
Well, since we can't see your database, I doubt we will be able to help you
much here.

You would need to share the query with us and the general structure of your
report.

Is the data showing up twice in the query, or just on the report?
