Remote Desktop web connection

I am tying to set up a remote desktop web connection for my home pc. I have it working great on the LAN but dont know how to access my pc from work (where I use a dial-up connection). My pc is connected to a router (netgear). Also what would I put in the address bar on the browser at work to make it resolve to my pc?? I am confused and in need of help. Thanks

You need to forward/open TCP Port 80 (IIS) and TCP Port 3389 (RDP) on your home router. You call
using the public IP of the home router.
