I highly recommend PowerQuest's Partition Magic for doing all the above. I
have never had it fail to do what it is supposed to do. You just install it,
make the dos boot disks and you can boot the program on start up, or use it
right from Windows.

Depends on which OS you're using. Prior to Windows XP, use a boot or
startup disk with Fdisk to partition and format command to format. Windows
XP install CD will let you partition and format. You can also use Disk
Manager in Windows XP to set your other partitions (if you only set one up
on install) after you have XP installed.

Boot up from the Windows XP CD to start its Setup. After installing various
drivers, the CD will off you the choice as to what Partitions you wish to
create, and offer to Format them for you. After you perform these tasks it
will continue with the XP setup on your computer.
