This is fairly easy to set up with just the column headings having the
fields you want.
Make sure for future use that the names and addresses go across (in rows),
not down.

They may look like address labels in Excel but will be nearly totally
useless. It may not look so pretty in rows but can be exported to any PIM,
CRM program, etc very easily and will also act as a mailmerge source in Word
for labels, documents, etc.

You can export your windows address book to a CSV file
I think you will see that as Export, other file and select the fields you
want.

My suggestion on order is, make the header row boldface as follows:

Phone#, Lastname, Firstname, address, town, State/Prov, Zipcode, email address

You might have a Home and work phone number, and address2, address3
that you have to fit in as well, and maybe even a home and work email .
of course if you have all that you will need to print in landscape mode.

This puts the phone number that you need to dial right next to the name,
when addressing a envelop you see the address next to the name without the
phone number between. Zip codes should be text (not numbers) that allows
you to sort them properly and 5 digit, zip+4 and zip codes from other countries
will all be left justified by default. Same for phone numbers make then Text,
and since area codes are required now, default left justification is better so you
can show an ext. number on right if needed. before area codes were req'd
right justification of text was better.
