random device USB connect / disconnect beeping

I am getting random beeping that a device is diconnecting and reconnecting
(like when you plug something into a USB port or unplug it). This can
happen 5 times in 5 minutes or not at all for a period of time.

Is there a way to see a log or something that would tell me what is
happening? Since this is not predictable, it seems kind of hard to cycle
through all of my USB devices to try and find the culprit.

I was watching the device manager while this was happening and identified
that something called the Generic USB Hub was disappearing and reappearing.

Under location is says: Location 0 (USB2.0 Hub Controller)

I have a DFI NFII Ultra motherboard. It has the USB ports on the back and I
have an internal USB / Media Card reader connected to the front header.
Finally, I have a Dell LCD monitor with 4 integrated USB ports. Any
suggestions on identifing the culprit?

Just unplug the monitor first, see if the problem goes away.
Then the card reader etc.

Its unlikely to be the read USB ports.

It might well be something as basic as a bad connection.
