Any ideas on what is causing this and how to fix it?

I've already created a new profile without any luck. I've tried
launching with all the /cleanthisandthat without luck.

The system is XP pro SP2 fully patched. Office 2003 SP3 fully
patched. Avast AV up to date and clean. HiJackThis shows nothing,
SpyBot runs clean.

Any ideas?

try disabling add-ins. Tools, options, other, advanced, addins.
