What is the most efficient way of creating questionnaires in Access? I have
hundreds of questions that I want to place on forms. The questions will be
stored in lookup tables and may use yes/no boxes or memo fields.

Well...

That's not it. Lookup fields are of VERY limited value, and multiple
yes/no boxes within a single record (or the whole idea of one field
per question, rather than one record per question) is improperly
normalized.
