For starters, this procedure would require that all your respondents have
Access installed on their desktops.
Then, when you got your responses back, you'd have to figure out how to
integrate them back together.
Surveys are much better run over a network (if available), or over the
Internet.
Surveys vary so much - starting with type of question, type of answer
required, and number of questions - it's hard to do a "one size fits all".
One company I've seen try it (for Internet surveys) is www.surveymonkey.com.
