Questionnaire data onto table

I wish to create a questionnaire and have Access automatically record the
responses on a table. I want staff to be able to use the same questionnaire
(i.e. it is refreshed for the next user) not to have to create new forms
for new users.
can anyone help with this - much appreciated

I had problems signing in so wasn't sure the first question ever made it -
