I am trying to construct a questionaire in word such that the questions and
identifying information can be compiled and tabulated. Some y/n, some scaled
responses, some ad lib text responses. I've tried some outlook survey code
and visited some survey websites... neither have worked for my needs. Any
guidance would be appreciated greatly.

At mousetrax.com you'll find a tutorial on how to construct simple forms in
Word. There's a built-in option that will save the form field entries as a
simple, delimited text file which you could import into a database.

Anything more, you'd need to use program code (macros) to extract the data and
insert it into a database. Not really a difficult task (IMO)...

You might also take a good look at InfoPath, if you're working in an Enterprise
environment. It sounds as if that would be exactly what you're looking for.
