qrcodes and vcards

We are looking for a way to tell someone using, for example, neoreader, on
their cell phone, to take an image of our QRCode and have it translate to a
Vcard. The Vcard should then install by itself into their contacts on their
phone. We can find several code converters to make QR Codes that translate
into the text file that a Vcard consist of, but we haven’t found a way to
make it install into the contacts on a phone. We have heard that Microsoft
has an app for this but we haven’t been able to find the information. Any
help would be appreciated.
 
What type of phone?

Outlook data is synced to smart phones either using activesync or the sync
software the phone uses. Ie, my blackberry uses the blackberry desktop
manager if I use the internet services or is synced over the air using
blackberry enterprise server.
