PWA form is not working properly

It was 2 computers with Outlook 2003: CA and CB.

On the computer CA I have installed not localized PWA component. Then I have
uninstall nonlocalized component and have installed localized Russian
component. Component working nice but form (that is a part of component) not
working. When I open this form, only bottom part of form is visible and
nonlocalized.

On computer CB I have installed localized PWA component and it's working
properly. Both CA and CB outlooks are targeted on one mailbox.

So I have try next steps:
1. I have delete form from outlook CA (menu Tools, Options, Other, Advanced
Options, Custom forms, Manage forms)
2. I have save to file form from outlook CB
3. I have installed form from file to CA outlook.
In result I have same picture: only bottom side of form is appear and non
localized

When I have try to view code of form on CA outlook (menu form, view code),
it's appear a error "Error of loading component. The operation completed
successfully"
When I try to view code on CB outlook, it's appear code of form.

How I can try to repair CA outlook?
 
Uninstall the add-in and verify the following;
-it is no longer listed in Tools-> Options-> tab Other-> button Advanced
Options-> button COM-Addins/Add-In Manager
-the form is no longer installed

Additionally clear the Forms Cache.

You should now no longer see any trace of the add-in and you can install the
localized version.

I have uninstalled add-in.
First, i have check
Tools-> Options-> tab Other-> button Advanced Options-> button COM-Addins
Then
Tools-> Options-> tab Other-> button Advanced Options-> Custom Forms ->
Manage Forms
Then press button "Clear cache"

Next, i have instaled component again and returned to same situation

Terminal Services? That's quite vital information you left out then. There
are quite some differences in behavior then especially concerning forms and
VBA.
