Publishing performance monitor data

A few years ago in an old job I was using a Freeware tool to publish
performance monitor data to a website (it put it in nice graphs). I can't
remember the name of the program I was using but I'd like to do the same
thing at my new place. I've been searching the web and can't find any clues.

Anyone know of a tool that does this?

I am not sure what kind of tool you have used to publish to the performance
data to the website.

According to my knowledge, SBS 2k3 has Performance and Monitor, which can
publish the performance data to the website and alert the administrator.

I have googled the Internet and find the following tool may can achieve
this goal. However, it might not be free:
