program to track health, medical, doctor's appt.s

Looking for a program that will allow me to record all of our family
medical history, doctor's appointments, etc. I don't want to enter all
of this info and has it accessible online, just a program to use at
home.....

Thanks so much..... I have tried Health Trakker, but it frequently freezes
up, or gives me an error which causes the most recent info to disappear. A
little frustrating... I'll give this one a try...
