Problems with logoff and shutdown

When I try to restart, shutdown or logoff of my Windows
session, the first attempt will fail, no error message or
anything appears. I make a second attempt and that one
works.

In other words, I have to shut down Windows twice before
any shut down actually happens.

This has been going on for months, and is driving me crazy.
