Did you contact your host? Does your host allow you to use FP Forms, some
don't.

To assist you in getting the best answers for FrontPage support see:

.

Ok, then how are you creating the form and then uploading it to the remote
server?

Try this, open the site on the remote server directly in FP, then File Menu
| New | Page | Feedback form, under form properties add your email address,
save and test.

Make sure the form does not have any server-side scripting code (such as
ASP, PHP, Perl, or SSI, etc.) on the page and that the page is .htm or .html

You will need to access the form via http://www.myserver.com/scvc/form.htm

If you form is clean of these issue, you will need to contact your host for
help.
