problem with publishing forms

I am trying to publish a form.

I begin by designing my form and publishing it to
Personal folders. I check the box which sends the design
with the form.

Unfortunately, when I send the form it seems that it
returns to the default message. What am I doing wrong?

When designing a form, you can choose to design either the compose form
(Form / Edit Compose Page) , the read form (Form / Edit Read Page), or both
together (Form / <uncheck> Separate Read Layout). In all likelihood, you
have designed the compose page, which means that only the person composing
the message (you) will see the form customizations. If you modify the read
page or both together, then both the composer and recipient will see the
form customizations, as long as the "Send Form Definition" box is checked on
the "(Properties)" tab. Note that if you customize other tabs (P.2) -->
(P.6), the form customizations will be applied to both the Compose and Read
pages. Try sending a form that has one of the tabs customized and see if
the recipient can see the pages. If not, you may be sending a form over the
internet, in which case you may need to set the "Internet Mail Format" for
the recipient to "Send using Outlook Rich Text Format." To do this, simply
double-click on the recipient's e-mail address in the TO line. If this
brings up a Contacts form, double click on the recipient e-mail address in
the e-mail field. Once you get an "E-mail Properties" dialog, choose "Send
using Outlook Rich Text Format" from the dropdown.
