Problem with publishing forms

I am try to create a custom form and email it to a colleague.

I go through the process of Tools, Form, Design a form. I choose a 'message' form and then customise it to suit my requirements. I then publish it to my personal forms library, taking care to check the box in Properties tab which "send form definition with item.

When I send the form to a colleague, this customisation is lost and they receive the default 'message' format.

What am I doing wrong?

Did you design a read layout or just the compose layout? Message forms have both by default.

What am I doing wrong?
Open the CONTACT ITEM for the recipient, select the email address, right-click it,
choose properties, and on the bottom of panel is a check-box labeled "Always send
to this recipient in rtf". Check that box. This ensures that the message is sent with
the TNEF wrapper.

For Outlook 2K2-3, the checkbox has been changed to a drop down list box.
