Problem with page numbering

I'm using the Page Number Format dialog box, but I can't figure out what
setting I should use to get a centered page number in the format "Page 1 of
10".

Also, I have a cover, introduction page and table of contents that I don't
want numbered, but the numbers start on the second page. Please help. Thanks
in advance.
