Problem making new contact in business contact manager

I tried to make a new business contact and whenever I click "New" I received
a message that said "The form required to view this message cannot be
displayed. Contact your administrator." When I go to make a new general
contact (not in business contact manager) I don't have any problems creating
a new contact. Also, if I am trying to open an existing contact in my
business contact manager I receive a message saying, "The custom form cannot
be opened. Outlook will use an Outlook form instead. The form required to
view this message cannot be displayed. Contact your administrator." Then,
when I click on "OK" I get another message saying, "Array index out of
bounds." Next, I click on "OK" and the contact is opened, but it is not in
the proper form for the business contact, it is the normal form for the
general contacts. Due to this, I cannot change appointments that are
connected to certain contacts, or attach any forms to the contact itself.
Can someone please help me figure out how to fix this problem?
Thank You,

BCM sometimes has what's known as a "forms cache" problem. Clearing the
cache usually fixes it. Choose Tools | Options | Other | Advanced Options |
Custom Forms, then click Manage Forms, then Clear Cache. If simply clearing
the cache doesn't fix the issue, try the other steps detailed at
http://support.microsoft.com/default.aspx?scid=kb;en-us;836558

The bulk of the forms cache issues in Outlook 2003 are fixed in Service Pack
1.
If you have not yet upgraded your version of BCM to V2, you can do so by
downloading it from:
