Printing to a Form Template

Hi
I'd like to be able to print to the fields of a standardized form that I have in PDF format. The form is a HCFA 1500, but could be anything, like a Form 1040. Any suggestions on how to set this up with the least amount of pain? Third party software, like Form filler? (Would prefer to avoid third party for redistribution reasons down the line

Hi,

I'm not clear how the Form fill did this, but you may first print your Form
into an image then draw the texts onto it like drawing on an image, finally
send the image to printer?

Of course you need do more job if you need display the input boxes on the
fields.

Does it suitable for your scenario?
