Right-click on the form in the datbase window, and select SaveAs. Use the
combo box to change the default "Form" to "Report". Rename it appropriately.

Starting point might be to open the form in design view.
Then from the File menu, choose Save As ... Report.
Then from the File menu, choose Save As ... Report.
