How should the monitor be positioned for optimal comfort? It doesn't
matter if its an LCD or CRT eh?

It will depend on the user. Usually the top of the screen should be about
level with your eyes. As the monitors have gotten bigger and using tower
cases , they are usually way too low.

.... depends on who you ask, some like to look down at their
monitor, some straight ahead, and those wearing bifocals are
pickiest of all. Where do you want it? I prefer center of
screen 3" below eye level, give or take.

I prefer it just above eye level to avoid the "hackers hump" you get from
looking down at a monitor for hours.
 
Yeah my monitor is a giant 19" LCD. It seems I have to move my eyes up
and down no matter what height I position it because its so big. But I
think looking down is less taxing on the body and eyes and I try to
keep my back and neck straight and just use my eyes.
