How can I monitor which ports are being accessed on the
server from the outside?

You could use "netstat -a" command run at server's command prompt.

Do a search in google for 'shields up' and go to that web
site. They have a nice scanner which runs on their server
which will scan your ports and tell you the status. It is
free. They also have some other great tools you will want
to check out.
