Pop-ups! Even with blocker set to high

I continue to get certain pop-ups, even with blocker set to HIGH. All other
pop-ups require the <CTRL> override. Malware? Virus?

Pop-ups include:
- http://ad.firstadsolution.com/rw?...
- Pacific Poker
- Adult Friend Finder
- http://isg21.casalemedia.com - Travelzoo
- and some other really irritating pop-up that 'congratulates' me with an
audio message

There is also a significantly noticeable drag in system and internet speeds.

Currently using McAfee Virus protection and AdAware with the latest updates.
+ Also scanned while in Safe Mode.
+ Tried 'SpyBot', but never found anything malicious.
+ Ran 'Hijackthis', but don't know what to do with the log file anyway.
+ Previously had 'Qoolaid' trojan, but that has been cleaned (even in the
registry).

WHAT NEXT?? I'm getting very frustrated that pop-ups (and pop-unders)
continue to interfere with my work online. How do I rid my system of this
malware without having to wipe the entire system?

User level: Advanced Intermediate (not expert)
