Please Help!!! Printing a field to a document.

Good Morning,
I would like to import data from a specific field into a Blank Demplate
document I have created. I have the fields Issue No and Issue Date in my
database and would like to export them in to the footer of the document.
I think I have made a good start by adding Merge Fields in to the document
with appropriate names. Where do I go from here?????
Ultimately I would like to click a button and have this done automatically.
