Please Help!? Linking Created Filtered Form to Reports

I am having a lot of trouble using a form to filter a report/query and then
print/preview the report by using command buttons. Heres what I am trying to
do:

Form is displayed:
Option button (Status) is selected then, either print or preview button is
clicked to print/preview the report.
Help would be kindly appreciated.
 
Where does a filter come from? What action by the user sets a filter? Are
there any controls and field names from the report's record source that you
would like to share? How about significant data types?
