Is there any way, in PowerPoint 2003, to highlight, bold, underline or
increase the font size of key words in the notes sections while presenting?
I like to have a full script of text to read prior to a presentation, but
during the presentation I'd like to quickly glance at the notes section and
pick out key words to remember to elaborate on for each slide.

Yes.

The notes section can be displayed either with or without text formatting.
Why they choose to default to without, I have no idea.

To show the bold/underline/italics (highlighting not available), you will
need to open the outlining toolbar. Counter-intuitive though it may seem,
the toggle button for formatting the notes section is on this seldom used
toolbar.
