This question does not appear to pertain to Microsoft Access databases, the
subject of this newsgroup.
Until the medical profession settles on a common format for medical records
information I fear the only answer is "there isn't one". Different medical
institutions use different software, different formats, even different naming
conventions, and there is no agreed-upon standard. This is a political and
business problem more than a technical one.
