PDF forms in FP2003

What's the easiest and most efficient way to add PDF forms to a web site?
I know this must be the MOST basic stuff, but I've never had to do it until
now.

Thanks for all help & suggestions.
I have adobe acrobat professional,
FP 2003
Win XP pro

handtohand.info is the site, and I need to add order forms and contract
forms.
