PC Fax Cover Sheets

I have deleted my fax cover sheet templates that came in my pc fax driver in
Office Word 2007 select cover sheet window. Can anyone tell me how to recover
them. I've tried the folder change option but it still does not place any
templates in the select cover sheet list.

I would like to recover what I have lost so that I can add cover sheets to
documents I choose to fax.
