password protect a form in access

I've created a report that is actually a form, that allows the user to choose
a month through a drop-down list.

I've locked and disabled all of the fields, so no changes can be made to the
form data.

How can I password protect the form so no one can go into design mode but me?

Access User Level Security will provide the *best* security for you,
but if you just need something simple for a single form and/or report
you could try these Microsoft KB articles:
