Parent/Guardian consent

will have summer custody of niece/nephew and sending to day-camp mon-friday
what consent forms should I have my sister sign? more then likely medical
release will be needed if either needs urgent care but should I have one
noting dates my sister has released into my care?
 
Wow, I think you have set a new standard in off-topic questions. I'm not
sure who you intended to be at the other end of this request but you have
found Microsoft Access database programmers ie: Geeks.

I see: "Public access forms". Now I understand!!!! ;-)

I think it might be a better idea to phone the camp.

This newsgroup is for questions about Microsoft's database
product, Access ...

to answer your question anyway ... many notary publics have
computerized forms they can generate -- what you are looking
for is a "Temporary Custody" form. Call around to notaries
in your area -- there will be a small fee for them to
generate the form. Many of them have computer programs to
"fill-in the blanks", then they print and notarize it. You
can probably prepare the form ahead of time and bring your
sister in to sign it when she comes. Make sure she has her ID.
