Outlook Forms

I am designing an Outlook form based on an appointment with a custom
page. I have published the form to a public folder. When a user uses
the form, the custom page is there, they fill it out but when it posts
to the calendar, the custom page is gone. I have gotten it to work
but it's hit and miss. Any suggestions? Thanks!

Btw, we're using Outlook 2003.

How is the form going to be used? Only to create items in the public folder?
To create appointments there and send them as meeting requests? To create
items in the user's own Calendar folder?
