View - Layout - check "Toolbar".

You are talking about on an Outlook Express "New Message" window. You
should have a menu bar on that window as there is no way to hide it.
It may simply be positioned where you can't see it. Maximize the "New
Message" window and see if you see the word "File" anywhere on one of
the toolbars. If so, grab it by the handle to the left of the word
"File" and drag it to its proper location at the top left corner of
the window. To grab it, click and hold on the vertical dotted line,
which is the handle.

As for the lack of a "Send" button, right-click on your button toolbar
at the top and choose "Customize". Highlight the "Send" button on the
left side and click the "Add" button in the middle.
