Outlook Client Hangs Constantly

Good morning,

Hopefully someone can help me on this. I have had
complains from some of my users that their Outlook keep
locking up. There are many different instances when this
happen. Some said it hangs when they try to open up
Outlook, some when they try to navigate through their
Inbox and some when they go to their calendar.

We are on Exchange 5.5 with Outlook 2000. Any help will
be much appreciated. Thanks!
 
For the hanging on boot-up try renaming outcmd.dat to outcmd.old. This will
reset the Toolbars back to default.

For the Calendar; is this while an item is open?

For the calendar, it is when the user click on it. It
will bring up calendar page, then the whole Outlook locks
up.

I created a custom contact form to be used by my entire organization
It basically uses standard contact fields with a few additional checkboxes and dates fields
The form works beautifully for everyone but it's creator - that's me - some gratitude
When I click on a contact, my CPU spikes 100% and locks up Outlook
I have tried all the switches suggested herein the group as well as the scanpst and detect and repair
Detect and repair appears to work for an hour or so
OS is XP current SP; Office version is 2003
