Outlook 2007 - List Box

I am trying to create list boxes for an Outlook form. I have added the
possible values on the Value tab in Properties and copied all items to the
Edit Read Page - the choices are availabe when running the form, but only the
first selection in the list is shown after e-mailing the form regardless of
the value selected when running the form. What have I missed?
 
The symptoms suggest that you have not published the form to the
Organizational Forms library or to each user's Personal Forms library, with
the "Send form definition with item" box on the properties page *unchecked*.

I have published the form to my Personal Forms Library and checked the "Send
form definition with item" box and still have the same results.
 
The "Send form definition with item" box must be unchecked for forms to work
fully. And you have to publish it either to the Organizational Forms library
