Is this user running a different build of Outlook 2002 than other users?
Check Help | About. Can they use other forms without problems?
Try deleting the entire contents of the Forms folder (the one containing
Frmcache.dat).

I have a user in the office that is unable to view the contents of our
organizational forms. After downloading the form only the "To" field is
displayed via the message tab and the document tab is also missing. We
have attempted to repair, uninstall and reinstall office and the problem
persists.
We have also attempted clear the cache, Logged on with a different profile
on her system, it worked. We have deleted her 'Outlook' profile, and
re-created, and this still did not work. Also deleted all temp files under
her profile and still no good.
