Organizing Start Menu

How can I organize the full Start Menu, moving individual programs into
folders grouped as I like, under WinXP Pro, as I did under Win98SE?

When I right-click "Start", then click "Explore" I see only a minimum of
program shortcuts and folders, unlike Win98SE where all my programs and
folders of programs appeared.
