The Open With command is not available with any file type,
whether or not the file is associated with a program.
I've tried holding the shift key, and a number of other
key-holds while right clicking on a file. I've tried
deleting file associations hoping the change would cause
the Open With to become available, but it didn't work.
I've tried looking through the registry, but nothing
appears to be out of the ordinary. Any help that can be
offered would be appreciated.

I figured it out on my own. In the registry editor under
HKEY_CLASSES_ROOT, the first entry is a * (for all files,
presumably). Under that, there is a shellex folder, then
under that is a ContextMenuHandler. In there, the Open
With key was missing. I added the new key and inserted
the following value to get it to work.

{09799AFB-AD67-11d1-ABCD-00C04FC30936}

I didn't have to reboot, but I did have to close Windows
Explorer and reopen it. When I did that, I had my Open
With menu back! Thank God!

I'll probably be the only person in the known universe to
ever have this problem.

Hope it helps someone else!
